My very good tank who wants to help make it better
Ok well here it is:
12+1 Tactics
10+2 Axe Mastery
8 Healing Prayers
Axe skills:
Cyclone Axe
Penetrating Blow
Tactics Skills:
Gladiator's Defense {e}
Bonetti's Defense
Healing Signet
Healing Prayers:
Mending
Healing Breeze
Restore Life/Vigorous Spirit (depending on if i'm soloing or teaming, Restore Life is groups, and Vigorous spirit for soloing)
I need to know what armor to get i'm thinking:
1 Knight's set piece
3 Gladiator's set pieces
Executioner's Helm (+axe mastery helm)

1.- is it for PVE? TOMBS? RA? TA?
2.- btw, mending in pvp is consideredby most as noobie, this is before flames start to appear on the build

Not even a couple points into strength? Even a 2+1 str is better than 0.

Seems a bit energy heavy to me. I would definetly drop Mending, even in PvE it has some pretty bad flaws. And, since you're going for high damage, you should put all your remaining point into Strength, no question.
Personally, I'd drop GD for Cleave.

If you don't mind the fact that you'll be doing basically no damage (ie pure tank) with this build, I guess it's fine. That means it is basically a PvE only build; there isn't really much use for a pure tank in PvP (unless you're planning on running life bonds or something on your monks, then *maybe* I could see some use for it).
I think you have too much healing, honestly. You don't need 2 defensive stance and 3 healing spells. If you want to try something a little more creative, how about something like holy veil instead of mending so you can hex remove? Or a purge conditions?

I'd help but you called it perfect, which means it can't get any better, which means I won't help you...
That and in my eyes, my W/N tanks far more efficiently than j00

I might replace Glad's Defense for Eviserate for PvP, and this build is pretty fair in PvE but in PvP I would guess you would get owned, no offense.

Ouch...double entendre.
I tend to define tank as a PvE-only character that soaks up damage. Usually as a Warrior I bring more than enough stances and armor buffs that I don't have any extra room on my skill bar for attack skills. It's not a tank's job to deal damage anyway.
For PvP I can't see this build come remotely close to doing anything useful. I'd just go adrenal spike and Eviscerate.
